Getting a Referral
Important insurance update for patients with
United Healthcare Medicare HMO or United Healthcare POS
If you are covered by either of these plans, a referral is required for any service in our office, including office visits, procedures, and surgery.

What you need to know
- Your referral must come from the primary care physician UnitedHealthcare assigned to you.
- Your primary care physician must send the referral to UnitedHealthcare before your appointment or surgery.
- If we do not have an approved referral on file, your appointment may need to be rescheduled, or you may be responsible for the cost of the visit.
- Please contact United Healthcare at 1-855-245-5196 to inquire about the physician to whom you have been assigned.
How to get your referral
- Call the phone number on the back of your insurance card.
- Ask who your assigned primary care physician is, if you are not sure.
- Contact that primary care physician’s office and request a referral to our office for your scheduled service.
